چکیده
A 54-year-old man with a medical history of diabetes, dyslipidemia, and abdominal aneurysm was seen for fever of 1 month’s evolution with no attributable focus. The physical examination was notable for high-pitched IV/VI systolic murmur at the mitral valve; the remaining examination was unremarkable. Laboratory findings included anemia (hemoglobin, 10.6), erythrocyte sedimentation rate, 80; and mildly elevated carcinoembryonic antigen (7.5). Hepatic lesions seen on ultrasound study and a positive blood culture for Streptococcus viridans suggested a diagnosis of fever with a tumor origin rather than endocarditis. Transthoracic echocardiography showed posterior mitral valve prolapse with severe regurgitation and no signs of vegetations. Transesophageal echocardiography revealed eversion of the posterior valve due to rupture of cords, with severe eccentric regurgitation toward the interatrial septum, where a 1-cm nodular image was visualized. Because of the atypical appearance of the mass, antibiotic therapy was IMAGE IN CARDIOLOGY

Motion of the interatrial septum in acute mitral regurgitation. Clinical and experimental echocardiographic studies.
The interatrial septal echocardiograms from 15 patients with acute mitral regurgitation due to ruptured chordae tendineae were compared with those from 14 normal subjects. On the cross-sectional echocardiogram, the interatrial septal configuration in patients with chordal rupture showed a characteristic pattern in which the interatrial septum (IAS) was flat or slightly convex toward the left at...
متن کاملSystemic Embolization from an Unusual Intracardiac Mass in the Left Ventricular Outflow Tract
Endocarditis can affect any endocardial surface; in the vast majority of cases, the cardiac valves are involved. It is exceedingly rare to develop infective endocarditis on the endocardium of the left ventricular outflow tract due to the high velocity of blood that traverses this area. Herein, we present a rare case of left ventricular outflow tract endocarditis that likely occurred secondary t...
متن کاملDissection of intima on atrial septum patch after mitral valve replacement in a patient with infective endocarditis after incomplete atrio-ventricular septal defect repair: report of a case.
We encountered a very rare lesion, which included a dissection between a prosthetic patch for repair of an incomplete atrio-ventricular septal defect (AVSD) and a pseudo-intima which had developed. This is the first report of a dissection between an atrial septum patch and pseudo-intima after mitral valve replacement (MVR) in a patient with infective endocarditis following an incomplete AVSD re...

Infective endocarditis can complicate with ischemic stroke. Neurological complications develop in 20-40% of all patients with infective endocarditis. These are mainly due to vegetation embolism. This report represents a case of 30 years old female patient who presented with features of heart failure New York Heart Association (NYHA) class IV with infective endocarditis and left hemiparesis. On e...
متن کاملLeft atrial endocarditis as a rare complication of mitral valve endocarditis: a clinical case
BACKGROUND Infective Endocarditis (IE) is considered as a multifaceted problem in every aspect from etiology and presentation to diagnosis and management. Early recognition of this disease and especially its complications, remain a critical task for the cardiologist. Atrial endocarditis is a rare and sometimes unrecognized complication of mitral valve endocarditis. CASE PRESENTATION We presen...
